Idiopathic Cyclic Edema : Hormonal Studies , Albumin Kinetics and Electronmicroscopic Study of the Capillary Basement Membrane

Abstract

|
A case of idiopathic cyclic edema was studied in order to determine its pathogenesis.
A 21 year old female student, whose body weight fluctuated spontaneously from 43 kg to 55 kg cyclically with non-pitting edema, was found to have latent diabetes mellitus as proven by cortisone glucose tolerance test & serum C-peptide levels, minimal thickening of the glomerular basement membrane, decreased extravascular albumin fraction & the absence of the early component of the albumin distribution space. There was diabetic family history. Combination of furosemide and aldactone was temporarily effective in relieving edema. These results suggest that one of the important pathogenic factors may be an in creased capillary permeability with increased extravasations of plasma from intravascular space due to microangiopathy.
